Guidance Regarding a Deceased Employee

EMPLOYEE LIFE INSURANCE & BENEFITS

CHECK EMPLOYEE BENEFITS: Dayforce>>MyHR>>select employee name>>Payroll Elections or check the Earnings Statement to determine the employee's current benefits.

LIFE INSURANCE:
Check to see if the employee has Hartford Basic Life Insurance and/or Colonial Life Whole Life Insurance, if so, let the family know of the Employee's Insured amount that will pay out. Checks will only be made out to the listed beneficiary, not to the funeral home.  Once the death certificate is submitted, it takes about 30 days for the benefit to pay out. 

  - Contact the Archdiocese; Maura Dodson at 913-647-0362 to process the Hartford claim.  Please provide family contact information.
  - Have the family contact Colonial Life, if the employee has Whole Life Insurance. Colonial Life 913-451-1120

FAMILY HEALTH & DENTAL BENEFITS:
If family members are covered by active Archdiocese Family Health or Family Dental benefits, you are required to offer Continuation Benefits

401K TIAA ACCOUNT: This benefit would apply to Employees and Priests
If the employee was a full time employee, give the family the TIAA contact information. There will be a final contribution to the employee's account after the last paycheck, be sure and let the family know this.

                                      the Archdiocese Plan number for Priest 403b accounts is 406869

LAY PENSION PLAN:
If the employee worked 30 or more hours a week in the years before 2014, the employee may be vested in the Lay Pension Frozen Plan and the family may be eligible for survivor benefits. A copy of the death certificate will also need to be sent to the Chancery HR office 12615 Parallel Ave Kansas City KS 66109.

DISABILITY:
If the employee had an active disability claim with Hartford, please have the family call
Hartford at 800-898-2458 to notify them or you can contact Hartford as the employer.

FSA/Flexible Spending Account: 60 day deadline from the employee's date of passing
Listed FSA Beneficiaries should apply for unreimbursed FSA funds that have been set aside in the employee's Medical and/or Dependent Care accounts. Please be aware of the 60 day deadline to obtain these funds. Contact NueSynergy at 855-890-7239.
